My Schedule and Grade Record

Dr. Diane Thompson, NVCC, ELI

Use this form to keep a record of your progress, personal schedule, and the grades you receive in this course. You will receive your personal Start, Last Refund and Blackboard Sign-in, Last Withdrawal and End Dates in the mail from ELI. Print out this page and fill in the due dates for each unit counting from your start date as week 1.

Note that the final Unit is longer and more demanding than the earlier ones, and allow more time for it, as indicated on the schedule.

As you submit work, fill in the date you submitted it and the grade you received for each assignment. Then, to know where you are in the course, just add up the grades. Once I grade a piece of work, I will not re-grade it, so just go on to the next task.

It is important to keep copies of all your work until you have received your final grade, since I delete work from the forums after 30 days. You should also keep the e-mail grades I send you until you have received your final grade for the course.

Since I may delete work from the forums after 30 days, you must complete and post any revision in less than 30 days. I will not grade a revised essay without its draft and my comments on that draft.

My Start Date is:____________

My Last Refund Date and Blackboard Sign-in Date is:___________________

Students who do not begin their course by this date will be administratively withdrawn without refund.

My Last Withdrawal Date without an F is:______________.

If you do not successfully complete (grades of 60% or better) all of Units 1 and 2, including Exams 1 and 2  by this date, you will be dropped from the course with a grade of W.

My End Date is:_____________

NOTE: I have changed my  W policy starting summer 07. The reason is that too many students have been completing only Unit 1 for their W date and then not restarting work until it is too late to complete the course in time or even qualify for an Incomplete. By getting half the course work done by the W date, you will be well-prepared to complete the course in time, or at least qualify for an Incomplete by completing three quarters of the course by the end date.
