Student Grade Record and Personal Schedule

Instructor: Julie Peluso

Course Created by Dr. Diane Thompson, NVCC, ELI

Use this form to keep a record of your progress, personal schedule, and the grades you receive in this course. You will receive your personal Start, W and End Dates in the mail from ELI. Print out this page and fill in the due dates for each unit counting from your start date as week 1. Note that the final Unit is longer and more demanding than the earlier ones, and allow more time for it, as indicated on the schedule.

As you submit work, fill in the date you submitted it and the grade you received for each assignment. Then, to know where you are in the course, just add up the grades. Once I grade a piece of work, I will not re-grade it, so just go on to the next task.

It is important to keep copies of all your work until you have received your final grade, since I delete work from the forums after 30 days. You should also keep the e-mail grades I send you until you have received your final grade for the course.

Since I delete work from the forums after 30 days, you must complete and post any revision in less than 30 days. I will not grade a revised essay without its draft and my comments on that draft.

My start date is:____________

My W date is:______________. (If you do not submit any work by this date, you will be dropped from the course.)

My end date is:_____________

To get a print copy of your final grade, click here and follow the instructions for getting a receipt. Once you have completed the course, this receipt will include your course grade.
