COURSE EVALUATION

One of the most important sources of information we have about the quality of ELI courses is thoughtful student reactions.  We are therefore asking every student taking an ELI course to answer this short questionnaire.  Your responses and comments will be used to improve our entire program.  The answers should be marked on the green scantron sheet that was mailed to you with your course syllabus when you registered.

Please return the answer sheet to the Extended Learning Institute. The address can be found in the syllabus.  Write: "Attention Course Evaluation" on the envelope.  Thank you.

DIRECTIONS:

Use the evaluation answer sheet provided by lab personnel to answer each question.  Follow the directions to correctly mark your responses.  Use the blank space to make additional comments.
